REQUIREMENTS

An application that includes full name, affiliation, date and period of time should be sent by e-mail to Dr. Coralith García (coralith.garcia@upch.pe).

In case of research, the area of interest should be stated. If the application is accepted, an e-mail of confirmation will be sent.

If the participant decides not to come, we will appreciate if he/she sends an email at any time during the process.

An email confirming the date of arrival should be sent at least two-weeks before arrival.  A copy of passport (first page) and a photograph should be attached to this e-mail. 

After arrival, filling a form with personal information and one passport photographs will be requested.

 A white coat, stethoscope and other tools for physical exam should be brought.
